It selectively raises the level of acetylcholine in the cerebral cortex and hippocampus. Rivastigmine interacts with target enzymes and forms a covalent binding with them which temporarily deactivates the enzymes. The inhibition of cholinesterase can lead to a delay in the formation of fragments of the protein beta-precursor of amyloid and the formation of amyloid plaques that are characteristic for Alzheimer's disease.

In healthy young men, taking a dose of 3 mg orally reduces the activity of acetylcholinesterase in the cerebrospinal fluid by approximately 40% during the first 1.5 hours. The enzyme activity returns to the initial values at about 9 hours after reaching the maximum inhibition effect. In patients with Alzheimer's disease, the inhibition depends on the dose.

The medication is well-absorbed when taken by mouth. Its bioavailability is around 30-40%. The maximum concentration in the blood is achieved within one hour of intake. The active substance is metabolized by the hydrolysis with the participation of esterases. Then, it is mainly excreted in urine (around 99%) and feces.

The further rise of the dose to 4.5 and 6 mg taken twice a day is possible only in well-tolerance of the dose and not sooner than with 2-week intervals.

In the occurrence of negative reactions such as nausea, vomiting, pain in the abdomen, loss of appetite or body mass, worsening of extrapyramidal symptoms such as tremor, it is advised to skip one or two doses. If the adverse effects do not fade away, it is advised to lower the dose back to the previous one if it was well-tolerated or suspend the treatment.

If the treatment had been suspended for more than three days, it is needed to resume it with the dose of 3 mg split into two 1.5 mg intakes. Then dose elevation should be made as described above.

The optimal maintenance dose is selected individually and is usually in the range between 3 and 6 mg taken twice a day.

To achieve the maximum effect, it is recommended to take the highest well-tolerated dose. The maximum recommended daily dosage is 12 mg split into two intakes.

The therapy can continue as long as it provides the beneficial effect. A doctor responsible for the patient must regularly assess the effectiveness of the medication. If after three months of the therapy the symptoms of dementia do not diminish, the treatment must be discontinued. Besides, it is needed to revise the reasonability of the therapy if there are no signs of recent improvement of the patient’s condition even if there had been an improvement before.

It is impossible to predict the individual reaction to the therapy with the drug. However, it is statistically proven that the best effect is achieved in people with Parkinson’s with moderately manifested dementia or visual hallucinations.

There are no clinical data on the therapy longer than six months.

Interactions of Generic Rivastigmine with other medicines

In general, the simultaneous use of the preparation with drugs that affect cholinergic structures is not recommended.

If the medication is used along with beta-blockers especially cardioselective ones, there is a risk of enhanced heart side effects. Therefore, it is not recommended to take Exelon along with beta-blockers.

What side effects can Generic Exelon cause?

The drug use can cause the onset of such adverse effects as increased fatigue, dizziness, headache, sleepiness, agitation, depression, tremor, nausea, vomiting, diarrhea, pain in the stomach, loss of appetite, heartburn, allergic reactions, increased sweating, and others.
