Erectile Dysfunction: The Warning Sign Men Ignore

Most men treat this as an embarrassment to be solved privately and quickly. It is worth reframing, because the reason it matters medically has very little to do with sex.
An erection is a vascular event. It requires arteries to widen and deliver blood on demand, which makes it one of the most sensitive tests of blood vessel function that exists — and one that reports its results without anyone ordering it.
🔴 This is the single most important thing on this page. The arteries supplying the penis are roughly one to two millimetres across. The arteries supplying the heart are two to three times wider.
So when the lining of the arteries begins to fail throughout the body — which is what atherosclerosis is — the narrowest vessels show it first. Erectile difficulty commonly appears two to five years before a heart attack or stroke in the same man.
That makes new erectile difficulty a cardiovascular warning, not merely a sexual problem. Guidelines in several countries now recommend that a man presenting with it should have blood pressure, cholesterol, blood sugar and cardiovascular risk assessed as a matter of course.
If you take nothing else from this article: ask for those tests. Getting a prescription without them treats the symptom and ignores the message.
🚫 The combination that kills
Before anything about how these drugs work, the absolute prohibition.
Erectile dysfunction medication must never be taken with nitrates. Both lower blood pressure, by different mechanisms that multiply rather than add, and the result can be a collapse in blood pressure that is fatal. This is not a caution to be weighed. It is an absolute contraindication.
Nitrates include tablets and sprays for angina, patches, and isosorbide mononitrate or dinitrate. They also include nicorandil, and the recreational inhalants sold as poppers, which contain amyl nitrite and are frequently used in exactly the situations where these tablets are taken.
Two practical points. Nitrates are prescribed for chest pain, so a man with heart disease may be taking one without connecting it to this warning — and one such medicine, isosorbide mononitrate, is stocked in a general pain category rather than a cardiac one, which makes the connection easier still to miss. And the interval matters: after taking a tablet for erectile dysfunction, nitrates must be avoided for at least 24 hours for most of them and 48 hours for the longest-acting one. Anyone who develops chest pain in that window must tell the emergency team what they have taken, because it changes the treatment given.
Alpha blockers for prostate symptoms, such as those covered in the article on prostate enlargement, also lower blood pressure and require care with timing and dose rather than outright avoidance.
🔍 Why it happens
| Cause | Detail |
|---|---|
| Vascular disease | The commonest cause overall. Smoking, high blood pressure, high cholesterol and diabetes all damage the vessel lining |
| Diabetes | Damages both vessels and nerves, and produces ED earlier and more severely than most other causes |
| Medication | A very common and reversible cause. See below |
| Low testosterone | Less common than assumed. Suggested by low desire as well as difficulty, plus fatigue and loss of muscle |
| Psychological | Anxiety, depression, relationship difficulty and performance anxiety, which frequently layers on top of a physical cause |
| Neurological and surgical | After prostate surgery, in multiple sclerosis, spinal injury and Parkinson disease |
| Sleep apnoea | Strongly associated, commonly undiagnosed, and treatable |
| Alcohol and smoking | Both directly implicated, and both reversible |
Whether the cause is primarily physical or primarily psychological has practical consequences, and the article on that distinction covers how to tell — the short version being that erections on waking, or during masturbation, indicate the machinery works.
💊 The medicines that cause it
Worth checking before assuming the cause is permanent, because this category is reversible and is missed constantly.
Antidepressants, particularly the SSRI class, are among the commonest culprits — described in the article on escitalopram, where the point is made that most people stop the medicine silently rather than raise it. Adjusting dose, switching drug, or adding treatment for the specific problem are all ordinary options.
Beta blockers and thiazide diuretics for blood pressure, where alternative classes frequently cause less difficulty.
Finasteride, taken for hair loss or prostate enlargement, causes sexual side effects in a proportion of men, and for a minority these persist after stopping — an outcome now reflected in product information in several countries. Anyone considering it for cosmetic reasons deserves that information beforehand rather than afterwards.
Also: spironolactone, antipsychotics, opioids and, over time, heavy alcohol use.
⚙️ How the tablets actually work
They do not create an erection. They amplify the body natural response by preventing the breakdown of the signal that keeps blood vessels open.
This means arousal is still required. Nothing happens without sexual stimulation, and a great many reported failures come from men who took a tablet, waited, and expected a result. It is worth stating plainly because the misunderstanding is common and the disappointment leads people to abandon a treatment that would have worked.
| Practical point | What to do |
|---|---|
| Timing | Most take 30 to 60 minutes to work. The longest-acting option can be taken well in advance and remains effective for up to about 36 hours |
| Food | A heavy or fatty meal substantially delays and reduces absorption of the shorter-acting ones. The long-acting one is much less affected |
| Alcohol | A small amount is fine. More impairs the response directly and defeats the tablet |
| Number of attempts | Try at least four to eight times before concluding it does not work. Success rates rise substantially with repeated use as anxiety falls |
| Dose | If the full dose fails after several proper attempts, the answer is usually a different drug or investigation, not a higher dose |
The three main options differ mostly in timing rather than in how well they work. The shorter-acting ones suit planned occasions; the long-acting one suits spontaneity and is also available as a low daily dose, which has the additional advantage of treating prostate symptoms at the same time in men who have both.
Never take more than the recommended dose, and never combine two of these drugs. Higher doses increase side effects without improving results, and combining them multiplies the effect on blood pressure without any evidence of benefit.
⚠️ Side effects, and the two that are emergencies
The usual ones are predictable from the mechanism, since blood vessels widen everywhere rather than selectively: headache, facial flushing, blocked nose, indigestion, and with the long-acting option a characteristic back and muscle ache appearing a day later. Some men notice a blue tinge to vision with one of them, which is harmless and temporary.
🔴 Two situations require immediate care.
An erection lasting more than four hours, particularly a painful one. This is not a joke and it is not something to wait out — trapped blood becomes deoxygenated and the tissue begins to be damaged, and permanent loss of function follows if it is not relieved. Attend an emergency department rather than waiting for morning.
Sudden loss or reduction of vision or hearing, in one or both. Stop the drug and seek urgent assessment.
Also seek help for chest pain during or after sexual activity, which needs assessing on its own terms and where you must say what you have taken, since it determines what treatment can safely be given.
🏃 What works besides tablets
These are not consolation advice. Several have measurable effects on erectile function itself.
Stopping smoking improves it, sometimes substantially, and the vascular benefit accrues everywhere else at the same time.
Regular aerobic exercise has genuine trial evidence for improving erectile function, working through the same vessel-lining mechanism that makes ED a cardiovascular marker in the first place. The article on exercise covers why those benefits appear well before any change on the scale.
Weight reduction helps, as does reducing alcohol.
Treating sleep apnoea, which is strongly associated and very commonly undiagnosed.
And where anxiety is a major component — which it frequently becomes even when the original cause was physical — psychological treatment works, and works better still combined with medication for the first months, because success breaks the cycle that failure created.
🔎 A word about where these come from
This is the most counterfeited drug class in the world, by a wide margin. Analyses of tablets bought from unverified sources have repeatedly found the wrong dose, no active ingredient at all, or contaminants including printer ink and industrial compounds.
The practical implication is simple: know the manufacturer, the stated strength and the active ingredient of what you are taking. A product that does not name all three is not one to swallow, and a dose higher than the licensed maximum is a reason for suspicion rather than a selling point — higher doses do not work better, and they interact with blood pressure more dangerously.
🩺 When to seek help
- An erection lasting over four hours — emergency
- Sudden loss of vision or hearing — stop the drug, seek urgent care
- Chest pain during or after sex — emergency, and say what you have taken
- Taking any nitrate, nicorandil, or using poppers — these tablets are absolutely contraindicated
- New erectile difficulty — ask for blood pressure, cholesterol, blood sugar and cardiovascular risk assessment
- Difficulty that began after starting a new medicine — frequently reversible with a change
- Low desire as well as difficulty, with fatigue and loss of muscle — testosterone worth checking
- Curvature or pain on erection — a separate condition needing specialist assessment
- No response after several correct attempts at a full dose — investigation rather than a higher dose
- Snoring with daytime exhaustion — possible sleep apnoea
Treatment is stocked in the mens health category. Caverta (sildenafil 100mg) and Cenforce (sildenafil 100mg) are shorter-acting and taken about an hour beforehand, away from a heavy meal. Cialis Soft (tadalafil 20mg) and Cialis Super Active (tadalafil 20mg) act for far longer and are less affected by food. Levitra (vardenafil) is a third option. All carry the same absolute prohibition with nitrates, and none should be combined with another of them or taken above the licensed dose.
❓ Frequently asked questions
Why is this a warning sign for the heart?
Because the arteries supplying the penis are only one to two millimetres across, while those supplying the heart are two to three times wider. When the artery lining begins to fail throughout the body, the narrowest vessels show it first — so erectile difficulty commonly appears two to five years before a heart attack or stroke in the same man. Blood pressure, cholesterol and blood sugar should be checked.
Which medicines must never be combined with these tablets?
Nitrates, absolutely and without exception — angina tablets and sprays, patches, isosorbide mononitrate or dinitrate, nicorandil, and the recreational inhalants known as poppers. Both lower blood pressure by mechanisms that multiply rather than add, and the result can be fatal. After taking one of these tablets, nitrates must be avoided for at least 24 hours, or 48 for the longest-acting one.
Why did it not work for me?
Most often one of four reasons. These drugs amplify a natural response rather than creating one, so sexual stimulation is still required. A heavy or fatty meal substantially delays the shorter-acting ones. Alcohol beyond a small amount defeats them. And most importantly, success rates rise considerably with repeated use — try at least four to eight times before concluding it does not work.
What is the difference between the options?
Mostly timing rather than effectiveness. The shorter-acting ones take 30 to 60 minutes and suit planned occasions but are affected by fatty food. The long-acting option works for up to about 36 hours, is much less affected by food, and suits spontaneity — it is also available as a low daily dose, which treats prostate symptoms at the same time in men who have both problems.
Should I take a higher dose if the normal one fails?
No. Above the licensed maximum, side effects increase and the effect on blood pressure becomes more dangerous, while results do not improve. If a full dose has genuinely failed after several correct attempts — with stimulation, away from a heavy meal, without much alcohol — the answer is a different drug or investigation of the underlying cause, not more of the same.
Could my other medication be causing this?
Very possibly, and it is a reversible cause that gets missed. Antidepressants, especially the SSRI class, are among the commonest. So are beta blockers and thiazide diuretics for blood pressure, where other classes often cause less difficulty. Finasteride for hair loss or prostate enlargement causes sexual side effects in a proportion of men, occasionally persisting after stopping. Also spironolactone, antipsychotics and opioids.
When is an erection an emergency?
When it lasts more than four hours, particularly if painful. Trapped blood becomes deoxygenated and the tissue starts to be damaged, and permanent loss of function follows if it is not relieved. Attend an emergency department rather than waiting for morning. Sudden loss of vision or hearing is the other emergency — stop the drug and seek urgent assessment.
Does anything besides tablets actually help?
Yes, and with real evidence rather than as consolation. Stopping smoking improves erectile function, sometimes substantially. Regular aerobic exercise has trial evidence behind it, working through the same vessel-lining mechanism that makes this a cardiovascular marker. Weight reduction and cutting alcohol both help, and treating sleep apnoea — strongly associated and commonly undiagnosed — can change the picture entirely.
Is it in my head?
Usually it is both, and the distinction is less clean than people expect. Erections on waking or during masturbation indicate the machinery works, which points towards a psychological component. But anxiety commonly layers on top of an originally physical problem, which is why medication and psychological treatment together work better than either alone in the first months — success breaks the cycle that failure created.
How do I know what I am actually taking?
This is the most counterfeited drug class in the world, and analyses of tablets from unverified sources have found wrong doses, no active ingredient, and contaminants including printer ink. Know the manufacturer, the stated strength and the active ingredient before swallowing anything. A dose above the licensed maximum is a reason for suspicion rather than a selling point, since higher doses do not work better and interact with blood pressure more dangerously.
